Liability Release and Waiver form: *

I am the parent and/or legal guardian of the child named below above and I consent to their participation in the design and critical thinking workshops conducted by IECI LLC (doing business as Curious and Creative Studio). I am aware and understand that these workshops teach concepts rooted in creative design are taught through hands-on model-building activities that may include the use of various materials and tools including but not limited to paper, card, scissors, craft knives, glue sticks, hot glue-guns, paints/markers, foam and wood craft blocks. I understand that this may present inherent and ordinary risks, including but not limited to minor cuts, slips/trips/falls/burns, injuries, and contact with allergens/irritants. Notwithstanding the risks, I acknowledge that I am knowingly and voluntarily consenting to the child's participation in the activity with an express understanding of the risks involved and agree to accept and assume, on behalf of myself and the child, any and all risks arising from the child’s participation in the activity, whether caused by the ordinary negligence of Curious and Creative Studio or otherwise. I hereby expressly waive and release, on behalf of myself and the participating child, any and all claims against Curious and Creative Studio, its owners, employees, staff, and volunteers, from any and all claims, damages, liabilities or losses, whether arising out of the ordinary negligence of Curious and Creative Studio or otherwise. I covenant not to make or bring any such claim against Curious and Creative Studio or its owners, employees, staff, and volunteers. This waiver and release does not extend to claims for gross negligence, willful misconduct, or any other liabilities that any applicable law does not permit to be released by agreement. I also grant permission for photos and videos of me and/or my child taken during the workshops to be used for promotional, websites or educational purposes by the Company. I expressly waive any right of privacy, compensation, copyright, or other ownership right connected to me. I understand I can withdraw this consent anytime by notifying the organization. I further agree to indemnify and hold harmless Curious and Creative Studio from any and all claims which are brought by or on behalf of the participant, which are in any way connected with participation by the child in the workshop. BY SIGNING, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTOOD ALL OF THE TERMS OF THIS RELEASE AND CONSENT TO THESE TERMS.
